- 中文摘要: Vecuronium Bromide為一具極低治療濃度之非去極化神經阻斷藥。本研究應用紫外光暨電化檢測原理及高效能液相層析法,快速、精確的測定vecuronium在人血或血漿中之濃度。檢體處理方式是先酸化,再以液體-液體抽取法抽取。以類似結構的tubocurarine為內標準品。電化檢測器是以Ag/AgCl電極設定其工作電位為+0.65V及+1.05V。當以vecuronium血中濃度與波峰面積比(vecuronium/tubocurarine)作圖,在vecuronium濃度範圍為25-500 ng/ml 呈線性關係,且其相關係數大於0.997。臨床上可能造成干擾的藥物如atropine,apresoline,droperidol,fentanyl,labetalol,thiopentone,atracurium和valium均經測試證實不會干擾vecuronium,和tubocurarine之測定。 此分析方法已成功地應用於手術中病人vecuronium的藥物動力學之研究。此法之最低血中測定濃度為3 ng/ml。Vecuronium為一二式室藥物,其分佈及排除半衰期分別為3分鐘和93分鐘。
- 英文摘要: Vecuronium bromide is a non-depolarizing neuromuscular blocking agent with a rather low thera-peutic level. Rapid, sensitive, and selective determination of vecuronium bromide in human blood orplasma was essential for pharmacokinetic study. We developed such a method using HPLC with electro-chemical detection. Samples were first acidified, followed by a one-step liquid-liquid extraction. Tubo-curarine, which has a structure similar to that of vecuronium, was used as the internal standard. Theelectrochemical detector, Ag/AgCl electrodes, was operated by setting the working electrodes, W1 and W2, at +0.65 V and +1.05 V, respectively. When vecuronium blood concentration was plotted versus peak arearatios (PAR) of vecuronium over tubocurarinc, a linear relationship was observed over the range of 25ng/mL to 500 ng/mL with a correlation coefficient greater than 0.997. Clinically possible sources of inter-ference, such as atropine, apresoline, droperidol, fentanyl, labetalol, thiopentone, atracurium, and valium,were examined and none showed interference in the assay of vecuronium and tubocurarine. This method has been successfully applied in a preliminary study of the pharmacokinetics of vecuro-nium in a patient undergoing surgery. The low detection limit of this method in the patient was 3 ng/mL.
