中醫脈診脈象圖形化之研究

木報告是以圖形類比的方法認定脈波中各線段、點、曲面、斜率及角度在座標軸上的變化。儘可能引用西方科技及醫學術語來溝通中醫的脈學。從一千三百餘人次的脈波圖象中用類比法歸納出十種重點變化作為認定的準則。用以選出由三種不同脈象相互搭配變化而衍生的圖型,一一與中醫典籍中所述二十九脈之定義相互印證。這批模型可作為將來電腦程式研究的數值依據。在圖形整理的過程中,已經看出脈波變化隱藏著疾病的線索,例如舒張期抖顫(diastolic shudder)之與肝病、水鎚脈(water-hammer pulse)之與肺病及心臟病、重搏脈(dicrotic pulse)之與心律不整(arrhythmia)及肺部病變、P段變化與藥物及情緒變化等。惜乎脈象似乎受生物晝夜韻律(circadian rhythm)之干擾而影響其穩定性,故疾病診斷之統計暫時不能完成。再者元滑壽診家樞要的舉、按、尋(浮、中”沉)的手法自P是西方Poiseuille定律所導出的血管內外壓力差(transmural pressure),本實驗用之以觀察脈象之變化,確有其特異性。此種特異性是否為體內臟腑之訊息將是有趣的展望。

In this article we divided the pulse waveinto 10 main criteria of change by its amplitudeand slope, and also classified basic models ofpulse wave by its integral morphology. For thepurpose of communication between occidentaland oriental medicine some terms from specialfield of cardiac catheterization were introducedand some occidental diagnostic term with theirequivalent meaning in ancient orthodox Chinesemedicine were put together here. About sixteen characteristic patterns weresampled by reviewing 1,300 different pulse re-cords in our laboratory. Those patterns fit thedefinition of ancient Nei-Ching and is beingexplained by modern science. However the in-stability of pulse wave hindered its way to suc-cess. In Nei-Ching the circadian concept wasmentioned which might be the cause of the in-stability of the pulse wave. The follow-upstudy of pulse wave records in different timessuggested this view so, but it requires furtherstatistical evaluation.
