- 中文摘要: 利用IBM(Cobe)2991細胞處理器,吾人已能簡單、迅捷、有效地分離大量骨髓取得血沉棕黃層細胞(共16例)。分離後之容積約僅為原始量的八分之一,而同時仍能保存 90% 以上數目的有核細胞。將之置入特殊之冷凍袋中混以 TC-199組織培養基及最終濃度為 10% 之 DMSO 作為細胞的冷凍保護劑(共9例)。解凍後血液幹血胞以 CFU-GM 檢測其恢復能力平均為 50.39 ± 19.54%,此一結果適於臨床應用。 至今(1987年10月31日),已有三例血液惡性疾患病人在超致死劑量的化學一放射線治療後,輸以自體冷凍保存之骨髓以解救其致命之骨髓毒性。兩位急性非淋巴性白血病,第一次緩解之病患,已在移植後無病存活了分別178天和157天,而一位何杰金氏病第四期,第三次緩解之病忠,則在移植後第59天因本態性間質性肺炎併發症過世,當時白血球造血機能恢復良好。 初步的實驗室及臨床結果令人鼓舞,自體骨髓移植值得進一步研究發展,尤其是對那些因年紀太大或無組織配合抗原 HLA 相合之兄弟姊妹供給骨髓,以進行異體骨髓移植的病患,此種模式提供了一個很好的加強化學-放射線治療的機會。
- 英文摘要: A simple, rapid and effective technique using the IBM (Cobe)-299l cell processor for the concentra-tion of buffy coat cells from large volume marrow has been well adopted (n=16). Only about one-eighthof the original volume was obtained while retaining more than 90% of the total nucleated cells to be cryo-preserved in polyolefine bags with TC-199 culture medium and final 10% dimethylsulfoxide (DMSO)(n=9), processed by a computerized Nicool ST-20 (France) programmed freezer and stored in a vaporphase of liquid nitrogen at -196°C. Stem cell assay by CFU-GM after thawing yielded a mean of 50.39±19.54% which has been satisfactory for clinical implementation. So far, three cases with hematological malignancies had been rescued by autologous cryopreservedmarrow after supralethal doses of chemoradiotherapy. Two patients with acute nonlymphocytic leukemiatransplanted in lst remission as of Oct 31 had been disease free for 178+ and 157+ days, respectively,after transplant which was taken at the corresponding age of 53 and 42 years. The other patient who wasa victim of Hodgkin's disease, stage IV, and was transplanted in 3rd remission, expired on the 59th daybecause of the complication of idiopathic interstitial pneumonitis despite excellent granulocytopoieticreconstitution. The preliminary results are encouraging for further exploitation, especially for those who wouldotherwise be candidates for allogeneic bone marrow transplantation but are limited by age or lack of anHLA-identical sibling to serve as marrow donors.
