清代基層地方官人事嬗遞現象之量化分析

1. Amniotic fluids obtained from 245 nor-mal and 45 complicated pregnant women ad-mitted to the National Taiwan University Ho-spital are studied to find out the relationshipbetween the analytical data and the fetal maturity(gestational age and birth weight of the new-bbrns) and evaluate their value for clinical ap-plication in the assessment of fetal maturity. 2. Amniotic fluids are collected either byabdominal amniocentesis, by Drew-Smythe ca-theterization, by amniotomy or at the time ofelective cesarean section. 3. Lecithin and sphingomyelin are measuredby thin layer chromatography and determinedeither by visual or densitometric reading. Bothplanimetric L/S ratio and densitometric L/S ratiocorrelated well with the gestational age andbirth weight of the newborn. 4. The L/S ratio is a reliable indicatorfor fetal maturation or age but not of lungmaturity: since only one neonate developed re-spiratory distress syndrome in our series. 5. Amniotic fluid L/S ratios of complicatedpregnancies such as toxemia of pregnancy es-pecially eclampsia; premature rupture of themembrane; multiple pregnancy and some diabeticmothers show higher values as compared withthose of the same gestational age group innormal pregnancy. 6. Amniotic fluid contaminated with me-conium, blood or mercurochrome does not signi-ficantly influence the value of L/S ratio ascompared with the value of tioncontaminatedsamples from the same gestational age group innormal pregnancy. 7. L/S ratio has no significant differencebetween the different sex of the newborn. Typesof delivery and labor pain also, does not alterthe amniotic fluid L/S ratio significantly. 8. Amniotic fluid surfactant titers are me-asured by rapid surfactant test (RST) or foamstability test of Clements. The result is com-pared with either P ratio or D ratio andalso with gestational age and birth weight ofthe newborn. The RST does not correlate tothe gestational age and the birth weight well asthe L/S ratio does. It can only be used clinicallyas a screen test and only be taken into considers.tion for assessment of fetal maturity when itshows positive result. If it shows negative, itis meaningless. L/S ratio or other reliable me-thod should be applied. 9. Disappearance of bilirubin ( ) andincrement of creatinine in the amniotic fluidalso show good correlation with the fetal ma-turity. 10. Cyanophilic squamous cells in the am-niotic fluid obtained from pregnant women be-yond 36 weeks of gestation give the correctprediction (9%) of the sex of the fetus. 11. In, the assessment of fetal maturity byamniotic fluid analysis, combined method suchas creatinine, bilirubin and L/S ratio determina-tion seem more reliable th4n determination ofa single parameter.
