癲?病人長期服用抗痙劑對神經傳導速度之影響

研究長期服用抗痙劑治療癲癇病人之神經傳導速度,以服用diphenylhydantoin (DPH)年數分為五年以上及以下兩組,各包括50位病人。另選擇50位正常人為對照組。比較其四肢之正中神經,尺骨神經,腓骨神經及經骨神經傳導速度,與腓腸肌之H反射潛時。結果叢現癲癥病人四腫之運動及快速知覺神經傳導速度均較正常組為慢,遠端和近端傳導潛時及H反射潛時延長。且五年以上韋常比五年以下韋為慢。本研究並首度證實知覺神經傳導速度之減慢以波及快速知覺神經傳導為主。神經傳導速度與癲痛病人之年齡及服用DPH年數具統計學上意義之負相關·但與發病年數,DPH之血液穩定濃度無直線相關(P>0.05)。臨床上表現足踝深部肌腱反射.消失或服用多種抗痙劑病人其神經傳導速度亦有統計學意義之減慢。

Nerve conduction velocities of 100 chronicepileptics with long-term anticonvulsant the-ropy were studied .They were divided intotwo groups according to the duration of di-phenylhydantoin (DPH) therapy by using 5years as a landmark.Each group includes50 patients,another 50 normal persons werechosen into the control group. Theconduction velocity of median nerve,ulnarnerve,peroneal nerve and tibial nerve wascompared among three groups.Conclusively,the motor and sensory conduction velocitiesin each group of epileptics are slower thanthe normal control; the latencies of proximalor distal nerves and rreflex are prolonged.The slowing down of sensory conduction isconfined to the fast fiber. Also the group ofdrug therapy longer than 5 years is slowerthan the group of therapy shorter than 5years in nerve conduction velocities,thoughno difference in the comparison of evokedamplitudes.The slowing down of nerve con-duction velocities has a negative correlationwith patients' ages or duration of therapy,but no linear correlation with duration ofillness or steady serum DPH level. Amongthe epileptics of areflexia of Achilles tendon,the slowing down of nerve conduction velo-cities is significant than those with preservedreflex. Multi-anticonvulsant therapy (threecombination of anticonvulsants) also causessignificant conduction delay as comparingwith single DPH or DPH and phenobarbitalcombined therapy.
