- 中文摘要: 本研究乃利用兔子門脈部份結紮造成門脈高壓,探討其門脈血行動力學和部份可能相關之體液因子。經門脈部份結紮後,門脈壓力呈有意義之上昇,於第二天達於最高點。然後,下降但仍維持在門脈高壓狀態。使用電磁流量計來測門脈血流。門脈部份結紮後及第一天門脈血流沒有明顯變化,但於第二天後,就呈現有意義之增加。門脈阻力於結紮後,呈有意義之上昇,第二天下降,但仍維持比正常更高的門脈阻力。門脈內前列腺素E2和I2 (prostacyclin)在結紮後上昇,第二天後下降,但prostacyclin於第五天時又上昇。由此研究發現,一旦門脈壓力或阻力增加,會使得門脈內之前列腺素E2和prostacyclin之產生增高,藉著其會使血管擴張之功用,慢慢產生門脈系統之側枝循環和臟器充血。此門脈高壓症模型之病態生理,其起先致病機轉主要是由於阻力增加,但稍後階段,則是血流和阻力皆會增加,來共同維持門脈高壓。
- 英文摘要: Hemodynamic changes and certain humoral factors possibly responsible for splanchnic hyperemiain portal hypertensive rabbits created by partial portal vein ligation were studied. Portal pressure wassignificantly elevated after portal vein ligation and reached a climax on the second day. Then, it de-creased but still remained at a plateau in a portal hypertensive state in the following days. Portal bloodflow, measured with an electromagnetic flowmeter, had no significant change immediately after portalvein ligation and on post ligation day-1, but it increased significantly from the second day. Portal venousresistance rose significantly from the basal state immediately after portal vein ligation, then, it decreasedfrom the second day, but still remained at a high resistance level as compared with the normal (p<0.05).The levels of prostaglandin E2 and prostacyclin in the portal vein increased immediately after portalvein ligation but decreased on the second day. However, prostacyclin rose again on the fifth day. It isapparent from this study that once portal pressure or portal resistance increases, there is a surge ofprostaglandin E2 and prostacyclin in the portal blood. Due to the vasodilation effect of prostacyclin,prostaglandin E2 and other possible vasodilators, portosystemic shunts are gradually produced, and splan-chnic hyperemia develops in the portal hypertension. The initial mechanism of portal hypertension inthis model was due mainly to an increase in resistance. At a later stage, increased splanchnic bloodflow combined with increased resistance played an important role in the maintenance of portal hyper-tension.
