使用鈷六十遠隔治療機治療乳癌所造成骨髓劑量的研究

本文係探討乳癌病人接受來自鈷六十遠隔治療機的伽馬射線治療時骨髓劑量的測定。所使用的劑量計為自行研製的硫酸鈣(鏑)熱發光劑量計,假體則用特殊的侖道假體。研究到達全身骨髓的散射輻射重點為(1)治療室內散射輻射的能量分布,(2)侖道假體中散射輻射的衰減,(3)在腫瘤位置主射柱和散射輻射之間的關係。治療方法係根據過去二十年的經驗。本文又持別強調由於主射柱的入射角、照野、源至皮膚距離、腫瘤劑量等的影響。

This paper deals with the determination of the bone marrow dose for breast cancer patients treatedwith γ-ray from a (方程式無法摘錄) teletherapy unit. The dosimeters used were the self-fabricated thermoluminescentdosimeters (方程式無法摘錄) :Dy, and the phantom used was a special Rando phantom. The scattered radiation whichreached the bone marrow throughout whole body was studied with reference to(1)the energy distributionof the scattered radiation in the therapy room, (2) the attenuation of the scattered radiation in the Randophantom, and (3) the relation between the primary beam at the tumor site and the scattered radiation. Thetreatment method was based on the experience gained during the past two decades. Emphasis was placedon the effect to the bone marrow due to the incident angles of the primary beam, the field size, the sourceto skin distance, tumor dose, etc. The marrow dose thus obtained depends on excluding or including the portion under irradiationfield,and also on the way of treatment such as irradiation of internal mammary chain and chest wall, orirradiation of axillary supraclavicular and subclavicular lymphatics. The marrow dose may vary from mini-mum of 0.4 to maximum of 6.8% tumor dose.
