系統性紅斑狼瘡病人之免疫複合物

17位系統性紅斑狼瘡(SLE)病人,以免疫螢光法測定腎臟組織切片中沈積之免疫複合物,同時以polyethylene gly-col (PEG)沈澱法測定病人血中免疫複合物之含量。後者係以PEG沈澱物中所含免疫球蛋白為血中含量之百分率表示之。另以18位年齡及男女比例相近之正常健康人為對照組。 PEG沈澱物中免疫球蛋白總量百分率測知,17位病人血清中有13位呈(PEG試驗)陽性反應(76.5%)與對照組5.6%呈陽性反應有顯著差異(P<0.01)。其免疫複合物中含IgG, IgA, IgM, C3及C4占血中該成分含量之百分率,病人組分別為6.4%, 1.8%, 26.0%, 18.9%及36.3%;對照組為1.7%, 1.1%, 13.6%, 13.5% 7R 31.9%,除C4 外,其餘均有顯著差異(P<0.01)。以免疫螢光染色法測腎臟組織切片沈積之免疫複合物與以PEG法測得血中免疫複合物比較之,所得結果多數相吻合,尤以IgG及IgM免疫複合物之測定為然(82.8%以上之實驗數相吻合)。又病人血中免疫複合物含量之增加(平均值為7.4%,對照組2.7%)及血中補體成分比通常含量為減少,此變化自有臨床意義。病人血中免疫複合物經證明,主要是DNA-Anti-DNA複合物。此複合物之免疫球蛋白性質絕大部份為混合性免疫球蛋白(占92.3%),而以含IgG出現率最高,IgM次之,IgA又次之。

The immune complexes in renal biopsies and blood were determined by the immunofluorescenceantibody technique and polyethylene glycol (PEG) method respectively in 17 systemic lupus erythematosus(SLE) patients. The amount of immunoglobulins in the PEG precipitates was compared with that in theserum and expressed as a percentage. The control group consisted of 18 healthy subjects with close agesand sex ratio. From the results of total amount of immunoglobulins in the PEG precipitates, 13 of 17(76.5%) patients' sera showed positive PEG test. The difference was significant (P<0.01)in comparisonwith 5.6% in the control group. The immune complexes were found to contain 6.4% IgG, 1.8% IgA, 26.0%IgM, 18.9% C3 and 36.3% C4 in the patient group; 1.7% IgG, 1.1% IgA, 13.6% IgM, 13.5%C3 and 31.9%C4 in the control group. The differences were significant (P<0.01), except for C4. In comparison betweenthe localized immune complexes by immunofluorescence technique for renal biopsy and the circulatingimmune complexes by PEG method, most of the results were correlated especially for IgG- and IgM-im-mune complex determination (more than 82.8% tests were compatible). We found that the amount of im-mune complexes in the blood increased (the average value was 7.4% in patients and 2.7% in controls) andthe amount of complement components usually decreased in patients. The changes of amount of immunecomplexes and complement components in the blood are clinically significant. It is also proved that theimmune complexes contain mostly the anti-DNA antibodies (92.3%). Anti-DNA antibodies mostly aremixed immunoglobulins containing IgG, IgM, and IgA in descending order.
