閉鎖容積檢測與一般肺功能測定之比較研究

161名受測者,其中18名女性,143名男性,年?介於21至63歲間,受測者區分?三組,其一?健康而不抽香煙者計42 名,其一?平均?日抽煙24支以上者,計51名;另一組?具輕度支氣管疾病之症狀,例如:經常咳嗽、多痰者,計68名。 三組受測者分別受肺功能檢測及閉鎖容積檢測。比較三組人員FEV1 , MEFR , MMF , CV ,及CC之異常率。結果,吸 煙者FEV1, MEFR,SMMF之異常率分別?35% 、 32%及51% ,而CV及CC之異常率分別達88%及91% ,有輕度支氣 管疾病症狀者其FEV1,MEFR,MMF之異常率?46% 、 39%及72% .而CV及CC之異常率則達96%及98%,此結果 顯示,閉鎖容積檢測對於初期氣道阻塞性病變之診察,其敏感度遠比一般肺功能檢測包括FEV1,MEFR , MMF等?高 ,宜於使用?COPD早期診?之工具。本文中並求出國人閉鎖容積之正常?,其迴歸方程式? CV/VC%= (7.06+0.1l×年紀)±3.65 可用以預估標準?,可供臨床應用時之參考。

A comparison between conventional spirometric tests and the test of closing volume was conducted in group of 161 subjects, 18 females and 143 males, ranging in age from 21-63 years. The subjects were classified into three categories: the first group was 42 apparently healthy non-smokers, the second group was 51 regular smokers, and the third group was 68 subjects with mild respiratory symptoms. a new modification of Fowler's single breath technique was used to determine the airway closure. The frequency of abnormalities of the forced expiratory flow rate (MEFR), the maximal midex-piratory flow (MMF), the closing volume (CV) and the closing capacity (CC) were compared in the three groups. The incidence of abnormal FEVi, MEFR and MMF in the smokers was 35%, 32% and 50% respectively, while the incidence of abnormal CV and CC increased to 88% and 91% respectively in the same group. The incidence of abnormal FEV1, MEFR and MMF in subjects with mild respiratory symptoms was 46%, 39% and 72% respectively, while the incidence of abnormal CV and CC both increased to 96% and 98% respectively in the same group. The data indicates that measurement of closing volume is much more sensitive than the conventional spirometric tests. It is simple, quickly performed and considerably more sensitive than FEVj and MMF. The measurement of closing volume has proved to be well suited for use in early detection of small airway disease.
