大白鼠前壁及後壁心肌梗塞之心臟血管作用變化

大白鼠麻醉後開胸將冠狀動脈及靜脈分枝結紮以產生前或後壁梗塞。觀察一小時,兩天及七天後之血壓、心跳、心輸出量、總周圍阻力、心電圖、血漿腎素(PRA)及血漿電解質之變化。其結果前壁或後壁心肌梗塞一小時、兩天及七天後,其血壓及心輸出量顯著減低。總周圍阻力上升。後壁較前壁梗塞之這些變化顯著。一小時後心電圖ST變化顯著,但兩天後或七天後似有回復。PRA在前壁梗塞一小時後上升,但其血漿Na及K無顯著變化. 組織切片顯示二天後前壁梗塞占心室肌容積之35.4±2.0%,後壁梗塞為60.5±1.1%(P<.O1),但七天後前壁及後壁梗塞縮小而無顯著差別。

Hemodynamic and ECG changes following myocardial infarction were studied in anesthetized rats.The right or left coronary artery or the posterior coronary vein was ligated to produce the anterior orposterior myocardial infarction. Blood pressure, cardiac output and ECG were monitored 1 hr, 2 and 7days after the infarction. Blood pressure fell, heart rate increased and cardiac output decreased,while totalperipheral resistance increased in either early (1 hr) or late stage (7 days) of the infarction. These hemody-namic changes were more pronounced in the posterior than in the anterior infarction. ST segment of ECGelevated or depressed profoundly one hr after the ligation, but it depressed or reversed 2-7 days after theligation. Plasma renin activity increased from 2.45±0.50 to 4.67±0.57 ng/ml/hr one hr after the anteriorinfarction, while plasma Ne and K were found not significantly changed. Infarction size was larger in theposterior infarction (60.5±1.1%, n=11) than in the anterior infarction (35.4±2.0%, n=11, P<.01) asmeasured 2 days after the production of infarction, but the infarction area tended to shrink 7 days later.
